(2001) and &#8220;Valuing\nPeople Now&#8221; (2007) which required services to be planned to\nenable people with learning disabilities to lead full and\npurposeful lives.<span>&nbsp;</span> Other\ninitiatives include:</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\">\n<span>\n<span>&#183;<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span></span></span>\n<span>Local\ncommitments to developing services which are geared towards\nmaintaining People&#8217;s independence stemming from Our Health\nOur Care Our Say (2006);</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\">\n<span>\n<span>&#183;<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span></span></span>\n<span>Children\nand Young People&#8217;s Service Plan 2009-12 priories the need for\nan integrated approach to transition for vulnerable young people\nand those with learning difficulties and disabilities including\nmental health;</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\">\n<span>\n<span>&#183;<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span></span></span>\n<span>The\nHaringey Youth Strategy identifies the need to support young people\nwith disabilities in transition;</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\">\n<span>\n<span>&#183;<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span></span></span>\n<span>National\nTransition Support Programme (2008) has been established by the\nGovernment to raise the standards of transition in all local areas;\nand</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\">\n<span>\n<span>&#183;<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span></span></span>\n<span>Aiming\nHigh: Better Support for Disabled Children and their Families (May\n2007) is the Government&#8217;s transformation programme for\ndisabled children services. Transition support is one of the\nidentified work streams in the programme and emphasises the\nimportance of joined up working and services centred on children\nand their families.</span></p>\n<span><br />\n</span>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\"><span>9.2<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span>\nThere were some overlap with similar objectives, however the\nquality of outcomes was the most significant issue, some services\nwere provided through adult services and others through children\nservices; it was important to ensure that vulnerable children did\nnot fall through the gaps.</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\"><span>9.3<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span>\nIn response to a question regarding pooled budgets, it was noted\nthat currently there were no joint arrangements for children with\nphysical disabilities and mental health issues.<span>&nbsp;</span> There was joint procurement for\nthe Primary Care Trust and the Mental Health Trust.<span>&nbsp;</span> Clients were identified at age 16\nto ensure early identification of transition issues, consider\noptions for meeting the needs of the client and also to ensure\nvalue for money in service provision.</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoListBullet\">\n9.4<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span> It\nwas noted that the Audit Commission viewed Haringey as a high\nspending borough on commissioning and in response to a question on\nthe strategies taken by the Children &amp; Young People Service\n(the Service) to address this, the panel learned that the Service\nused a method called the Funding Calculator tool and were\nconsidering a Pan-London approach as a further possible tool to\nassist in proactively managing the Learning Disabilities market.\nHaringey have also been successfully working with a company called\nOLM to help review some of its expensive Learning Disability\nplacements and reduce costs.</p>\n<p class=\"MsoListBullet\">\n&nbsp;</p>\n<p class=\"MsoListBullet\">\n9.5<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span> It\nwas agreed that this issue would be discussed in more detail at a\nfuture meeting of the panel when budgets and value for money issues\nwould be discussed. Further details of the Funding Calculator can\nbe found at: <span><a href=\"http://www.southeastiep.gov.uk/\" class=\"external\">www.southeastiep.gov.uk/</a></span></p>\n<p class=\"MsoListBullet\">\n&nbsp;</p>\n<p class=\"MsoNormal\">9.6<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span> Due\nto increasing demand and budget constraints there was a need to\nplan services effectively to ensure value for money.</p>\n<p class=\"MsoNormal\">&nbsp;</p>\n<p class=\"MsoNormal\"><span>9.7<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span></span>\n<b><span>Definition\nof transition</span></b></p>\n<p class=\"MsoNormal\"><b><span>&nbsp;</span></b></p>\n<p class=\"MsoNormal\"><span>9,8\n<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span>\n&#8220;The <a name=\"_ftnref1\" href=\"http://www.minutes.haringey.gov.uk/#_ftn1\" class=\"external\" title=\"\"><span class=\"MsoFootnoteReference\"><span><span class=\"MsoFootnoteReference\"><span>\n[1]</span></span></span></span></a> [transition] process must be\nindividual to the needs and aspirations of each young person. It is\na fluid process, spread out over a number of years, and often local\noptions for young people are limited and support can be patchy and\ninconsistent. These challenges are compounded by young\npeople&#8217;s moves from one service to another at different ages.\nFor example a disabled young person may move from paediatric to\nadult health care services at 16, then at 18 moves from\nchildren&#8217;s to adult social care. This is alongside the\ntransition stages in their education. Each of these transitions is\nlikely to occur independently of each other, which means that young\npeople and their families may repeatedly have to deal with new\nagencies and professionals, re-telling their story each\ntime.&#8221;<span>&nbsp;</span></span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\"><span>9.9<span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span>\nY</span><span>oung people\nwho received children&#8217;s services may continue to need\nservices when they are adults. This will involve transferring\nresponsibility for assessing needs and providing services from\nchildren&#8217;s to adults&#8217; services. The process of\n<span>&nbsp;&nbsp;&nbsp;</span> transfer is\nreferred to as 'transition'.</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\"><span>9.10<span>&nbsp;&nbsp;&nbsp;</span> It was important that\ntransition was viewed as a process and not just a single event.\nImplementing improved transition involves: recognition of the\nimportance of the process; adequate consultation with professionals\nand users, flexibility in timing of transition; a period of\npreparation for the young person and family; information transfer;\nmonitoring of attendance until the young person is established in\nthe appropriate adult oriented service.</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\"><span>9.11<span>&nbsp;&nbsp;&nbsp;</span> The Panel learned that\nnationally young people with complex disability present particular\nchallenges because often there is no equivalent adult service able\nor willing to take on their long-term health care and medical\nsupervision. <span>&nbsp;&nbsp;</span>There is a need to develop\nand ensure that a holistic approach to the transition of young\npeople.<span>&nbsp;</span> However, there\nwas recognition that sometimes they may fall through the\n<span>&nbsp;&nbsp;&nbsp;&nbsp;</span>\nnet.<span>&nbsp;</span> The ADS Steering\ngroup were looking at developing a corporate model to address\nthis.</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\">\n<b><span>\n<span>9.12 </span></span></b><b><span><span>&nbsp;&nbsp;</span> Personalisation\nagenda<span>&nbsp;</span></span></b></p>\n<p class=\"MsoNormal\"><b><span>&nbsp;</span></b></p>\n<p class=\"MsoNormal\"><span>9.13<span>&nbsp;&nbsp;&nbsp;</span>\nPersonalisation of services was defined as &#8216;the way in which\nservices are tailored to the needs and preferences of\ncitizens&#8217; the overall vision is that the state should empower\ncitizens to shape their own lives and the services they\nreceive.<span>&nbsp;</span> This\ntransformation in the way that services are shaped and provided is\nbeing compared with the changes which occurred with the\nintroduction of the NHS Community Care Act 1990.<span>&nbsp;</span></span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\"><span>9.14<span>&nbsp;&nbsp;&nbsp;</span>\nPersonalisation varies from area to area but has the potential for\nservices to solve problems together; campaign\ngroup office for disabled people&#8217;s rights and many others.\nSpecial Schools have used enterprise schemes to provide work\nexperience for young people with complex needs and proceeds in identified the need to improve\ntransition<span>&nbsp;</span> for young\npeople with Additional Needs, Learning Difficulties and\nDisabilities, including young people with special educational needs\nbut without a statement, young people with mental health\ndifficulties and young people leaving care.<span>&nbsp;</span> There is also a need to ensure all\nplanning reflected the National Transitional Programme to raise\nstandards and to learn from best practice identified through this\nprocess and also the Personalisation Agenda. Greater emphasis also\nneeds to be placed on the key roles young people and their\nfamilies, separately and jointly have on this agenda.</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\"><b><span>9.24<span>&nbsp;&nbsp;</span>\nMy Service at 18</span></b></p>\n<p class=\"MsoNormal\"><b><span>&nbsp;</span></b></p>\n<p class=\"MsoNormal\"><span>9.25<span>&nbsp;&nbsp;&nbsp;</span>\nMy Service at 18 has been established as a joint strategic\ntransition planning group between C&amp;YPS and Adults LP, in the\npast each Directorate had its own strategic group with\nrepresentation from the Directorates on each, the new strategic\ngroup comprises representatives from Statutory and voluntary\nservices, parents and providers.<span>&nbsp;</span> The group had its first away day\nand agreed a joint action and delivery programme comprising of\nshort, medium and long term goals and will report to The\nDisabilities Forum, the Children&#8217;s Trust and to the Learning\nDisability Partnership Board also the Well Being Stream Board,\nlinking to the Haringey Strategic Partnership. </span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\"><span>9.26<span>&nbsp;&nbsp;&nbsp;</span>\nThe Group also identified the need to review the multi agency\npathways.<span>&nbsp;</span> The revised\npathways include short, medium and long term goals within a two\nyear timeframe.<span>&nbsp;</span> An\nexample is to reframe the fourteen plus panel to the Transition\nSupport Panel and to map out how all the Pathways are\nlinked.<span>&nbsp;</span> Clarification\nof all eligibility criteria and identification of key people in all\nagencies and clear roles and responsibilities would be\nrequired.</span></p>\n<p class=\"MsoNormal\"><span>&nbsp;</span></p>\n<p class=\"MsoNormal\"><span>9.27<span>&nbsp;&nbsp;&nbsp;</span>\nThe Panel were informed that</span> <span>Children and\nadult services have recognised the</span></p>\n<p class=\"MsoNormal\"><span>need to further\nreview and develop these services and this work had begun. The\nfirst step was the joining together of two transition steering\ngroups separately located in the CYPS and ACCS. (Adult, Culture and\nCommunity Services). The newly established steering group &#8220;my\nservice at 18&#8221; quickly identified four work streams;\nfollowing this an away day was organised to agree a <span>&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;&nbsp;</span>\nstrategic plan and delivery programme. This is alongside the\ntransition stages in their education. On 21<sup>st</sup> October 2009 Haringey Learning\ndisability Partnership hosted an Away Day to enable the Service to\nlook at the work they wished the Partnership board to undertake.\nScott Watkins, co- National Director of the Valuing People Team and\nDebbie Robinson, London Regional lead attended the morning session.\nDiscussion on the day included how to improve service for young\npeople and their families. Representatives of services such as\nhousing and leisure also attended. In addition parents who are also\nmembers of the Partnership Board and other carer forums now lead on\nservice developments and their work with Autistic Spectrum Disorder\nprovision is an example of this outcome.</span></p>\n<p class=\"MsoNormal\"><span style=\"q-family:\\\"HelveticaNeueLT Std\\\"\">&nbsp;</span></p>\n<p class=\"MsoNormal\"><span style=\"q-family:\\\"HelveticaNeueLT Std\\\"\">9.37<span style=\"mso-tab-count:1\">&nbsp;&nbsp;&nbsp;</span> A parent has agreed to\nkeep a transition diary for her 14 year old daughter with Down\nSyndrome.
